The Chronic Wound Service provides specialist care to people who have wounds that are not healing as expected. The Chronic Wound Service is a multi-disciplinary service for people with chronic wounds. The service provides:
The initial assessment usually takes about 90 minutes. At this time we aim to determine the likely cause of the wound and devise a management plan together with the patient. The next appointment will be determined after the initial assessment. The Chronic Wound Service team work collaboratively with community services to ensure patients receive the same wound management outside appointments with the Chronic Wound Service. We will fax a letter to the referring doctor after the patient's initial visit and again anytime there are changes to their wound management plan. We may also send a copy of letters to other community nursing or allied health teams.
